When coming into the office for an appointment we are asking that:
- We are only seeing children for Well Child Checks. For all other visits including sick visits we are scheduling Telehealth appointments.
- Everyone two years and older must be wearing a mask.
- Only one parent and one child for in office appointments.
- If anyone has been sick or exhibited any symptoms such as fever, rash, cough, or trouble breathing we ask that you call to reschedule.
CHADIS INFORMATION
You may receive an email or text message with an invitation to fill out a CHADIS questionnaire.
If your child has an asthma questionnaire, have them fill it out themselves starting at age five. Otherwise if your child is eleven or older, please have your child create their own account and/or login and fill out questionnaires themselves.
If you have ever filled out a questionnaire on your phone or on an iPad in our office you should already have an account and you may call our office for your username and to reset your password.
Your username may be your information in the format ‘Firstname.Lastname’ and there should be an option to reset your password. If this does not work or you have further trouble, please call our office and one of our staff will walk you through the process.
